Joe,
My sense is that you want.
A
1st ending
A
2nd ending
B
A
3rd ending as the end of the piece.
This would be accomplished with a “Da Capo al fine” at the end of the third A.
The 3rd ending would be noted as “fine.”

Hi Joseph,
I don't understand where your endings 3 and 4 go. But perhaps this could
help you:
{
\repeat volta 5 { a1 }
\alternative {{ b1 } { c'1 } }
}
{
a1
\set Score.repeatCommands = #'((volta "1."))
b1
\set Score.repeatCommands = #'((volta "2. + 5.") end-repeat)
c'1
\set Score.r

Hello Joe,
You talk of a third repetition, but show 1, 2, and 5...
