Looking at your screenshots, it seems that the kernel flows don't
reflect a switched that has "failed open" at all; for example, one would
never see a "controller" action in such a case. Can you show us the
OpenFlow tables just after such a failure and the ovs-vswitchd log from
the whole process?
Sorry, I forgot to attach the version information. I tried 1.9.3 LTS, 2.02
and 2.3.1 LTS downloaded from official website. All of them have the same
issue.

What version of OVS is this?
If the tap exists when you start OVS, then I'd expect it to work
without removing and re-adding. OVS doesn't currently notice
automatically when a device that was missing when it started up
appears. That would be a useful contribution, though, and I'd happily
accept a good implementation of such

do,
ovs-vsctl del-port , then ovs-vsctl add-port
(the "ip tuntap> command is used to create tuntaps on startup)

there an option with ovs-vsctl to have tap devices stored to reference

Not sure if this is a bug. There is a problem with a feature I've been
trying(openvswitch 2.1.0+git20140411-3) with "port mirroring". The main
issue I'm having with is that it "delays"(buffers with indefinite time
lapses) until a "threshold" of traffic is reached for it to "release".

Hello,
Assuming we have trivial case: libvirt, its QoS limits (for which
appropriate rules are executed for each port) and more than one VM
sharing the same OVS bridge. Port configurations are initially empty
and addition going through libvirt` internal command execution by
calling ovs-vsctl binar

Hey Ben,
Yeah, this is introduced by my changes. I'm working on a fix.
Alex Wang,

Well, I assume ovs-vswitchd dumping core. If not, you need to make it
dump core.
Once you have a core dump, invoke GDB on the ovs-vswitchd binary and the core
dump, then type "thread apply all bt" and paste the result into an email.

Hello,
after I add groups and flows via ovs-ofctl, for example:
ovs-ofctl --protocols=OpenFlow13 add-flow
unix:/usr/local/var/run/openvswitch/s4.mgmt 'in_port=2 actions=group:1'
When I exit mininet, it halts when it tries to stop the open vSwitch
switches.

I started writing one email when I had two bugs. I finished writing it by
doing a step by step through what I did. The first bug, which did break the
package, then finished, leaving me with this bug. Unfortunately, I forgot
to change the previous text.
So, how do I avoid the warning? I did some C
