TO REPRODUCE:
1. All Settings -> Keyboard -> Layout Settings -> Options...:
2. Numeric keypad layout selection -> ATM/phone-style
3. Click "Close"
4. Close the window.
Launch a Terminal:
Note that pressing "789" on the numpad does not result in "123". (standard US
keyboard)

Status in “gnome-control-center” package in Ubuntu:

I can confirm that this remains an issue in 12.04 Alpha 2 LIVE. There
is no obvious way for users to remove this icon if they wish to.

I can confirm this is still an issue in 12.04 release, x64. (Just in
case anyone was wondering.)
These should work in the meantime:
caps -> ctrl: setxkbmap -option ctrl:nocaps
caps -> ctrl and Programmer Dvorak: setxkbmap -layout us -variant dvp -option
ctrl:nocaps
caps -> ctrl and Programmer Dvo
