I've taken a screenshot of the alsamixer.
This configuration currently is the only way for me to get sound out of my
headset and not the speakers:

* Enable speakers, but put volume to 0, don't mute

Setting the scaling to 150% solved the issue for me. But I don't know if

combined with pngalpha and FirstPage settings. I've seen this on two
very different PDFs from different sources.
