I am running Etch. I can make an ISO using
# make-live -d etch --packages avrdude avrdude-doc avr-libc
but only one of the packages ("avrdude") is installed. The same thing happens
with this line in /etc/make-live.conf:
LIVE_PACKAGES="avrdude avrdude-doc avr-libc"

I just discovered how easy it is now to build my own live iso.
I managed to run it with Vmware (mounting iso file as the cdrom)
But I don't see how to put it onto my usb stick.
I am using live-helper 1.0~a12-1 and the following commands:
# rm -rf debian-live
# make-live config -d etch -b us

no, because at build time, the size of the target usb stick is not know.
however, as ben said, a flag like --usb-size could be usefull here;
which i'll add soon.

i build an img that is about 170 mb, i have a pendrive with 512 mb, when i do dd
if=binari.img of=/dev/sda , it create a partition of 170mb in usb, there are any
way to put the img using all the space in usb, and the partition will be
writable.

- i had to repackage live-initramfs-1.87.4 because of depends libc6
(removed dependency)
- /scripts/live-bottom: removed sed for event.d/tty* -> Unknow

I confirm that the image is indeed OK when run out of qemu.
I am using qemu on an amd64 installation and I have tested that the
problem appears both using the kqemu module and without it.
The qemu version is 0.9.0-2 from debian sid.
Does vmware have the same type of problems?
