After reading the Ubuntu bug report (#794642) I found that this Debian bug is
also due to the SATA mode in my computer being set to IDE. When this is changed
to AHCI the problem vanishes: /dev/cdrom appears.

After upgrading to Debian 6.0.3 I rebooted straight away, ending up with the
issue I mentioned. Rebooting again did not help, I had to run flash-kernel to
fix things...

FYI: the upstream fix for this issue has been pulled into the linux-next
tree. Not sure if this is upstream enough for the kernel team to backport to
3.0 or 3.1 yet, but I was able to cherry-pick the series into 3.0 with no
conflicts.

Good news everyone!
I found it that the problem is that the SATA controller, which is not in
AHCI mode. Adding setpci -d 8086:2828 90.b=40 in grub.cfg resolves the
problem.
