with a bit more searching I did manage to find the option to add in
the headers. Still can't get the postmaster bit sorted out though. Let
me search further :)

/usr/local/sbin/clamav-milter -ol $CLAMAV_MILTER_SOCKET
--postmaster-only --postmaster=not...@domain.com
