I did not put the hw:0,6, but hw:6.
alsa 1.0.11 is awesome.

I have an agere systems hda modem on my laptop. It
accessible through alsa.
I have installed the last version of slmodemd, compiled with
alsa. I alsa installed alsa 1.0.11, which has full support
for the modem.
If I write aplay -l the modem is correctly identified as
card: 0, d
I write aplay -l. The modem is correctly identified as
card: 0, device :6, ATI HDA SB, SiS 3054.
I launch slmodemd, I use the command:
slmodemd --alsa hw:0,6 --country=UK
SLOMDEMD identifies the modem, and creates tty/SL0. I then
create a connection in kppp.
