this part you describe is what does not work:
*click the blue "+" button in Accounts, enter in some details and click
"Save and Continue". This should create a test account under your manager
account.*
the second page of the details it asks for simply says "There is no billing
setup

I double checked, it most definitely says "Level: All" but there are still
no accounts.

Update for you, i checked the next day and while the ad accounts still do
not appear here:
they do show up on the overview page:
