Do a "q libv" on this volume and verify it's in the correct slot. Or

run an "audit libr checkl=barcode" and see what happens.
The tape may not be in the correct slot or heaven forbid, on the floor

This one is easy. You never ran a LABEL LIBV on A00328; the hint is in
the fact that the header is not readable per the error messages. The
