This has the feeling of a hardware problem OR
scsi interface/drivers/firmware which are not compatible.
Is that combination of hardware/firmware/driver code supported
/up_to_date
I would check on the latest firmware for the library.

I agree.
Just use the AIX install cd/dvd and follow the
instructions to reload via sysback.
We use a cronjob to backup our machines
via sysback on a weekly basis. Then use the client to
backup data daily.
