The docs on www.smartcardsys.com are pretty much it unless you can find the
non-existant documents Microsoft claims to have on their site.  There is
currently no skeleton IFD_Handler but you may use the one included with the
PC/SC resource manager ( ie: Reflex, Litronic, Towitoko modules )  I made a
quick IFD_Handler ( doesn't support everything - just what it needs ) that sits
on top of a ctapi.  I will probably release a skeleton IFD_Handler once I
figure out a clean way of handling all those TLV's the reader must communicate.
