Okay, so example.sagetex.sage is created. That leads me to think that the script you're using to run Sage on that file isn't working in the correct directory. Are you using TeXShop? How are you trying to run Sage? You might try putting "pwd" into that script by itself on a line; that will print the directory the script is working in.
