You can make a tax-deductable donation over the web as follows:

 (1) Go to 
https://secure.gifts.washington.edu/as_mathematics/gift.asp?source_typ=2&source=EAV

(This is linked to from http://www.math.washington.edu/Development/giving.php
at the UW math dept page.)

(2) Click on "Friends of Mathematics Fund", then click "Continue" (which
is above the check boxes).

(3) Fill out the form, and in the comments box write that 'the donation is for
  "The SAGE Foundation"'

You should receive a receipt after you submit the form.
