Bingo.  Found the dumbass responsible for THAT idiocy.

Original key is D major (sharp), new key is Ab.  Ab is one of those 
pesky "black key" notes and the moron who wrote this assumed if the 
original key was sharp, the new key should be sharp, so it's going for 
G# instead of Ab.

What a moron.  I'm going to fire that McIntyre guy immediately, and go 
fix his mess.  Surely this explained the other thing I saw with the 
double flats too.

I'll have to ponder on this one, and try to get it right this time.
