Thank you for your reply. I missed it in my inbox (too many
mails). Saw your reply on the redhat-devel-list archive. The
problem was solved by restarting the sshd server. I'm now happily
running openssh-2.3.0p1-14 together with pam-0.74-4.

Which version of openssh-server are you running? The kbdinteractive
support we put into Raw Hide shortly after the last errata had a problem
with the auth2-pam module (which kbdinteractive uses) which was fixed a
couple of weeks ago.
If it helps, I'm not seeing this with pam 0.74-4 and openssh-server
2.3.0p1-14, both from Raw Hide.
