I think you could just use shrink wrap tubing from an electronics supply
house or hardware store.  I sorta remember that stuff rattles a little
(though it was on a all-three-cables-routed-top-tube bike a friend had).

I use three sets of 3 rubber doughnuts - one at each end near the ferrule
and one in the middle.  If you leave each set of three unseparated, they
don't migrate as much.

I also bought a roll of this stuff some time ago, especially for two bikes I
no longer have, one of which had headtube-mounted cable guides.  It works,
stays stuck and isn't obstrusive, but I'm not sure I'd want to lay it the
length of the top tube.  I've been told you can find similar product in auto
parts stores.
