I don't want to completely derail this thread but it's tending in this 
direction so I'll fire away:

We have a local, custom shoe manufacturer (Loveless) who started out making 
orthotics and who now seems to be the shoemaker to the stars.   Very 
high-end custom stuff with exotic leathers but they also have a basic 
leather shoe starting at $650.  I have very wide feet (6E often doesn't cut 
it), high arches, high insteps and I'm developing a hammertoe on my right 
foot.  I also have a Morton's Neuroma in the same foot and a few miles on a 
bicycle results in pain (I ride in one of the stiffer Crocs because all 
other shoes don't have the toe room I need) so a set of custom dual-purpose 
(work and cycling) shoes is looking more and more attractive.  Is $650 a 
reasonable price or is it on the higher end for custom shoes?   I also have 
to consider the fact that this guy is local and can do hands-on 
measurements of my feet instead of sending me a foam mold.   

It sounds like some folks here have experience with custom shoes so i would 
love to hear your experiences and opinions.
