I'll second the AirCaddy/ShipBikes recommendation. I've used them 4
times, without any issues. The shipping crate they sell ($100)
survived 3 round-trips before needing a replacement cardboard shell
($40).  The crate is designed for minimal dis assembly of the bike.
For many bikes you only need to remove the front wheel and seat, and
rotate the handlebars. I was also able to fit my helmet, shoes, tools
and bags in the crate.
