One of the mistakes that mechanics often make is that they don't go through all of the steps in order. Shifting is a system, especially with indexing, and it helps to be systematic. Here's the steps in a quick summary: 1. Check limit screws 2. Check cable tension 3. Adjust indexing (barrel adjusters) It is also important to know whether your rear derailleur is a traditional (high normal) or rapid rise (low normal) style. The adjustment procedures are a bit different for each. High normal derailleurs are in the highest gear when the cable is relaxed. Low normal derailleurs are in the lowest gear when the cable is relaxed. You'll find the difference in adjustment discussed at the end of the Park article. One last thing that I didn't ask yet, and it matters with indexed systems, is whether you switched out the chain and cassette with the same system that you had before. If you changed out the cassette for the same number of gears you had before, great. If you decided to "upgrade" from 8 speeds to 9 without changing the indexing rear shifter, then that can cause the type of problem you are describing.
