Anne,

My guess is either the barcon for the rear is loose and so in effect
shifting on it's own, or the cable is not taut enough.  For the first, try
tightening the screw that holds the lever - its pivot point.  For the
second, turn the adjusting barrel out (counter-clockwise as you look down
the cable into the barrel) to increase cable tension or reattach the cable
to the clamp on the derailleur.  If I were going to do the latter, I'd give
the cable a good stretch by pulling it away from the bike before releasing
it from the clamp.  Hope one works.
