I actually just had the same thing happen to me. I caught it before
the chain actually broke but one of the plates had come off the pin as
you describe. I think it was caused because one of the screws holding
the rear rack on was too long and was actually going right through the
eyelet (or whatever you call it) on the dropout and hitting the
outside plates when the chain was on the smallest sprocket.

Does that make sense? Had your son, by any chance, just screwed a rack
or fenders on?
