To follow-up on myself:

I configured xorg.conf to identify the two devices and defined a single
screen on each.  With this configuration, ratpoison v1.4.9 works fine
with the two monitors on the first card.  The other monitor is found by
the X server and I can move the mouse over there.  It is controlled by
another server (e.g. one has DISPLAY=:13.0 and the other :13.1).  I can
explicitly start up an xterm, say, on the third monitor by typing a
command elsewhere but I cannot get focus over there.

If I try to start a window manager over on the 3rd monitor, it doesn't
work: ratpoison says "there can be only one" ;-) and fluxbox simply says
that a window manager is already running.

Also, if I move the mouse over to the 3rd monitor, it cannot be moved
back.

It would seem that ratpoison has lost some functionality with multiple X
servers?
