That is Qmail-Scanner's way of telling you that either the message was
too big to be acceptable for scanning by SA, or that SA took too long to
scan it. From the sounds of it - it's the latter.

As the timeout is 30 seconds, it means you have a problem with SA. Check
your load, network performance, etc. All sorts of external factors can
make spamd take longer to scan messages than is acceptable.

Actually - I see you are running an old release of both SA and
Qmail-Scanner. You should really upgrade both first.
