Dear Yuval:

i'm not an emminence in this area, so i haven't the necessary knowledge to 
answer your question with totally authority... take this as a commentary from 
other user.

The remote SMTP sends to your SMTP an e-mail, that is received. when this step 
is ended, then your SMTP moves the mail to the queue and only then starts the 
filtering process. In this concept, the communication between your SMTP and 
the remote SMTP has ended before the filtering starts, so the remote server 
can't give up with the sending process, because it's terminated with an ok 
status in a ok period of time (again, before the start of the filtering 
process).

i am new in qmail and in qmail-scanner, so i cannot answer your question, i 
just tell you how i understand the mail delivery and filter process (generic, 
for every MTA).
You may be need to read more documentation about this, so then you will have 
the authority to tell me "you have reason" or to tell me "you are wrong 
because ... (bla bla bla) ..."

now i will sugest to look for other posible cause.
