checked the queue on the pop/imap/webmail server, and there was his

keeps trying to send the message which it apparently thinks didn't go

What is 64.42.88.20 - is that your Qmail-Scanner server? Or is that
reported on your Qmail-Scanner server delivering to someone else?
I'm going to assume it is the Q-S server, in which case what could be
happening is that your AV/SA components are taking too long to finish
processing that 5M message (too long normally means >20 minutes) and so
the SMTP client drops the connection and retries. However, some minutes
later the AV/whatever finishes processing the first one and delivers it
- but by then there's another attempt happening, etc, etc.
There are some checks to stop that happening, so ensure you are running
Q-S 1.25 and check the qmail-queue.log file to find out where exactly
all the delay is occurring.
Then fix that.
