This is before execution of spam/virusscanners. The message of less than 2000 bytes long. Is this simply a slow incoming smtp connection, or is something else wrong?

Absolutely. In fact, it's been screwing up my stats so much that from the next release of Qmail-Scanner, it will deliberately reset its counter to zero AFTER the data has been received from qmail-smtpd. Before I was seeing some e-mails taking 200 minutes to process - when in fact it was a dodgy switch in front of the server severely slowing down throughput. The message would basically take 200 min to be delivered to disk, and then Q-S would take 3 sec to process - but stamp the syslog record as saying the message took 12003 sec "to process". Ewww.
