Err - "well of course not" I initially said to myself when I read your message - but maybe it's not so obvious to others. Think about it, if you're calling it with verbose_spamassassin, then you are saying "let SA rewrite the message - not Q-S". So you *shouldn't* be trying to fiddle with the Subject line via Q-S anyway. Disable that and configure SA to do that instead. As far as the duplicate e-mail goes, I think something else on your system is at fault. There is no known issues with Q-S in that area.
