I had a similar problem when I installed qmail-scanner. It turns out I didn't have the QMAILQUEUE patch installed and qmail-scanner wasn't getting invoked. After running test_installation.sh, I received 3 e-mails: a normal one, a spam, and an EICAR test. All went through fine and I received no alert e-mails. Is this your situation, or is qmail-scanner filtering but just not logging its activity?
