I'm not sure how you're running qmail.

On my installation, the QMAILQUEUE variable is called via the qmail-smtpd
run file.

You may want to try that instead of using the tcp.smtp.

Using this method, anything sent via smtp, inbound or out, is scanned.

And yes, I'm using vpopmail also.
