Have you checked /var/spool/qmailscanner/qmail-queue.log? I can not really help you further; I've had the same problems as you have and solved those, but now I'm stuck with a new one. If I get that one solved, I'll see if I can come up with some new ideas but since I'm going on Holiday tomorrow, this may take a week or two.
