As explained in the issue report's comments, the issue is due to the fact that the print layout contains 2 totally overlapping Map items ("Map 1" and "Map 2") with the same size and position in the layout's page but having different scale and extent.
In particular, you need to remove the "Map 1" Map item (which is actually empty and hidden under the "Map 2" map item) selecting it in the "Items" panel and pressing the Del or the Canc button on your keyboard or right-clicking with the mouse and selecting the "Delete Item" action.
Then you need to select the "Scalebar" item and link it to the "Map 2" map item (Item Properties -> Main Properties -> Map -> "Map 2") and then select the "North Arrow" picture item and link it to the "Map 2" map (Item Properties -> Image Rotation -> Sync with map -> "Map 2") .
