I have done some more experimentation.

I have outputted a geospatial PDF directly from the main QGIS window.  That’s 
seems to produce an accurate geospatial PDF in Adobe Acrobat Reader.

However when I export the geospatial PDF using a layout from the QGIS layout 
manager, that's where the problem with the scaling comes up.

I was referring to the QGIS's Measure Tool.

In any case, since the map's CRS is a UTM projection, there shouldn't be a big 
difference between the Ellipsoidal and Cartesian measurements in QGIS.
