I left it sitting there for 30 minutes to see if anything else appeared
on the console but it didn't.
I tried removing the HBA from the slot, then it can boot all the way to
the first question:

The petitboot code had some patch for a crash in mpt3sas, I don't know
if that is related to the mpii code:

Do you have any machine like this yourself or could it be useful to have
remote access to one of them via the BMC? Snapshots can be accessed
over TFTP.
Will the installer allow me to install to a USB external drive? I could
use that to test further stages of the installation without the HBA.
I captured all the output from the successful boot (without HBA) in case
it is useful
