I just tested it again using "ehlo myserver.com" in both upper and lower
case and both work. You have not shown any settings but i do not think
there are any settings that could disable the ehlo command but maybe
someone else more knowledgeable might know. See attached screen shot.

EHLO is an ESMTP command which I believe is blocked by that person
firewall (a Cisco Pix firewall with SMTP protocol fixup active).
Those stars in the headers are usually a sign that a Cisco Pix with SMTP
protocol fixup on is in front of a SMTP server.
This is supposed to "protect" the SMTP server by acting as some sort of
proxy and it does, AFAIK, amongst other thing turn off ESMTP commands...
It does barely nothing as far as protecting the SMTP server and causes
more problems than it solves...
