In my experience this generally results from two mail servers along
the path not communicating well. The first server doesn't get the
ACK from the second so requeues/retries the message, while the
second thinks all is well and sends the message on.

Assuming that that's the case, a way to figure out the offending
pair is to look at the headers of two (or more) of these messages
(received by the same recipient). Read the lines from the bottom up.
Where the lines diverge is where the machines aren't closing out
their transfer communication properly. If these machines are under
your control, look at the logs to see what is going on. If they
aren't, then track down their administrator(s).
