So my second guess was right. From that file:
postscreen_bare_newline_enable = yes
postscreen_non_smtp_command_enable = yes
postscreen_pipelining_enable = yes
Those *guarantee* that the first connection from any unfamiliar IP will
get a 450 reply to any RCPT offered.
