I think that Milter would be the way to go (no risk of losing email because
Postfix stays the "owner" of the message). There are Python and Perl
implementations, which is convenient if your filter is also in Python or
Perl.

Milters typically listen on a network port and want to accept connections
themselves, so they would not work under the spawn daemon which accepts
connections before creating a child process.
It's not possible to accept a connection in both spawn service and in its
child process.
