For me, sql errors are logged by proxymap. For example, when my mysql database is unavailable (like if I'm restarting it) I get: mail postfix/proxymap[87612]: warning: mysql query failed: MySQL server has gone away or mail postfix/proxymap[50060]: warning: mysql query failed: Lost connection to MySQL server during query -- I leave symbols to the symbol-minded - George Carlin
