It might be silly to you, but I have only been trying to configure postfix for about a month, and have to keep reading things like table(5) over and over again to be sure I understand what is what. So whilst now, it is obvious I was making an totally incorrect assumption and being "Silly" - it is not easy to figure out that I was being so until someone points out the error.

I still don't know why verbose logging didn't show a lookup of the table which then failed because there were no domains specified as keys. If it had I might have understood sooner, but I want to move on now so I am not going to let it bother me.
