That "same code" is commented out in the Postfix 2.11 "post-install"
script. It was only present in 2.10 and some early 2.11 snapshots.

There is no version of Postfix that added the smtpd_relay_restrictions
safety net when an explicit non-empty value was present in main.cf.
The server in question had the "safety-net" value added by something
other than Postfix "upgrade-configuration". And certainly not by
Postfix 2.11 where the code to do so is NOT present.
