Here:
# postmap -s btree:verify | sort | uniq | wc -l 
104785
# postmap -s btree:verify | awk '{print tolower($1)}' | sort | uniq | wc -l 
104654

That's 0.125% difference, so it gets worse the bigger the site gets :)
