The answer is in your mail server logfile: the client submits the
same message multiple times; the recipient is on an alias that is
expanded multiple times; or Postfix delivers the message multiple
times, perhaps because the remote SMTP server is too slow.

If the above is in not your logs, then message duplication can happen
after Postfix has delivered the message.
