Your webmail application submits mail via sendmail(1), not via SMTP.
Postfix smtpd_*_restrictions are only effective on SMTP mail.
Either update your webmail application to a version without
exploitable bugs or disable the account that has been compromised.

The version of horde applications that i use is the last, i write to
the list reporting the problem and not get responds, so where i can do?
the account with uid= 48 belong to apache
apache:x:48:48:Apache:/var/www:/sbin/nologin
let me find if is possible the horde webmail application can submit
via smtp messages.
