If I send a message to a non-procmail enabled account, it delivers to a mbox file in $HOME/Maildir/ with a name like msg.Dv4Z.

Ah, no.  I did home_mailbox = Maildir/

All mail is stored uner the user's own home. mail_spool_directory is for mbox files, not maildirs.
