Bingo, RHEL5 comes with Postfix 2.3.3 and doesn't have support for mysql/pgsql tables - which are kinda popular, y'know. You can go and get the centosplus package that includes those, but then of course any updates will clobber it, and... That's probably why a lot of the "how to setup a complete mail system" guides I've seen are written for Debian-type systems. :)
