this generally means that the client and server got un-synchronized
(client sends a command but server is not in the state to accept it).
there are many cases when this could happen, either because of a bug in
the client or the server, or in a proxy/firewall/router/... in the path
between the two.

you can try finding if the messages that get such errors have something
special. but that will be hard.
