guess what. (almost) all spam I get has a Subject header.

you will get better advice if you
- show what restrictions you have put in place
- a sample "missed" spam (put full headers and body on http://pastebin.com).

start with

        reject_rbl_client zen.spamhaus.org

if after this you get other spam, then feel free to ask (but post a
sample on pastebin.com).

you also may want to add this header check:

/^X-Mailer: %WORD/      REJECT fix your ratware and try again
