One of the possible causes are of a "not meaningful" mirroring tecnique. An example for this is to enable both in/out mirroring for each port. This will lead to dupes. Each packet transiting through the switch will be mirrored at both the incoming port and the outcoming one. In this case it is adviceable to mirror just either incoming or outcoming packets on all ports. Hoping this might help. Let me know how things get evolved.
