Start with checking that the install is OK. There should be 3 new files in the Addons directory called iPlayer.opml iPlayerParser.pm iPlayerPlayableParser.pm
If the 3 files are present - make sure the permissions and ownership of the files are the same as the other files in the Addons directory. Also you could check the actual file lengths against the file lengths in the zip file to make sure files are not corrupted/truncated. If you change permissions / ownership / files contents - you should restart SC before retesting. If it still doesn't work - set logging for formats.xml to WARN and then select an iPlayer menu until you get the error message.
